消栓通络胶囊联合丹参川芎嗪注射液治疗脑梗死恢复期的疗效观察

摘    要:目的探究消栓通络胶囊联合丹参川芎嗪注射液治疗脑梗死恢复期的临床疗效。方法选取2013年1月—2016年1月在天津中医药大学第一附属医院就诊并接受治疗的脑梗死患者95例,随机分为对照组(47例)与治疗组(48例)。对照组静脉滴注丹参川芎嗪注射液,10 m L加入生理盐水250 m L,1次/d。治疗组患者在对照组的基础上口服消栓通络胶囊,6粒/次,3次/d。两组患者连续治疗2周。比较两组患者治疗前后临床疗效、NIHSS与ADL评分、血脂水平、血液流变学指标差异。结果治疗后,对照组和治疗组总有效率分别为78.72%和91.67%,两组比较差异具有统计学意义(P0.05)。治疗后,两组患者NIHSS评分显著下降、ADL评分显著升高,同组治疗前后比较差异有统计学意义(P0.05);且治疗组NIHSS与ADL评分显著优于对照组(P0.05)。治疗后,两组患者三酰甘油(TG)和总胆固醇(TC)水平均显著下降,高密度脂蛋白(HDL)水平升高,两组比较差异具有统计学意义(P0.05)。且治疗后治疗组患者血脂水平明显优于对照组(P0.05)。治疗后,治疗组患者全血黏度、红细胞积压、血浆黏度比治疗前均显著下降(P0.05),且治疗后治疗组患者血液流变学指标明显低于对照组,两组比较差异具有统计学意义(P0.05)。结论消栓通络胶囊联合丹参川芎嗪注射液能改善恢复期脑梗死患者血脂水平,降低血液黏度,减少神经功能缺损,具有一定的临床推广应用价值。

Abstract:Objective To study the curative effect of Xiaoshuan Tongluo Capsules combined with Salviae Miltiorrhizae and Ligustrazine Hydrochloride Injection in treatmen of cerebral infarction at convalescence. Methods Patients (95 cases) with cerebral infarction at convalescence in the First Teaching Hospital of Tianjin University of TCM from January 2013 to January 2016 were randomly divided into control (47 cases) and treatment (48 cases) groups. Patients in the control group were iv administered with Salviae Miltiorrhizae and Ligustrazine Hydrochloride Injection, 10 mL added into normal saline 250 mL, once daily. Patients in the treatment group were po administered with Xiaoshuan Tongluo Capsules on the basis of the control group, 6 grains/time, three times daily. Patients in two groups were treated for 2 weeks. After treatment, clinical efficacy, NIHSS and ADL scores, blood lipid levels, and hemorheological indexes in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control and treatment groups were 78.72% and 91.67%, respectively, and there was difference between two groups (P < 0.05). After treatment, the NIHSS scores in two groups were significantly decreased, but the ADL scores were significantly increased, and the difference was statistically significant in the same group (P < 0.05). The NIHSS and ADL scores in treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). After treatment, the TG and TC levels in two groups were significantly decreased, but HDL level was significantly increased, and there were differences in the same group (P < 0.05). And the blood lipid levels in the treatment group were significantly better than that in the control group, with significant difference between two groups (P < 0.05). After treatment, whole blood viscosity, erythrocytes overstock and plasma viscosity in the treatment group were significantly decreased (P < 0.05), and the hemorheological indexes in the treatment group were significantly lower than that in the control group, and the difference was statistically significant between two groups (P < 0.05). Conclusion Xiaoshuan Tongluo Capsules combined with Salviae Miltiorrhizae and Ligustrazine Hydrochloride Injection can improve blood lipid levels, decrease blood viscosity and neurological deficits in treatmen of cerebral infarction at convalescence, which has a certain clinical application value.
