孕康糖浆联合地屈孕酮治疗复发性流产的临床研究

目的探讨孕康糖浆联合地屈孕酮治疗复发性流产的临床疗效。方法选取2014年1月—2016年1月在新疆医科大学第二附属医院进行治疗的复发性流产患者94例,根据治疗方案的差别分为对照组(47例)和治疗组(47例)。对照组患者口服地屈孕酮片,10 mg/次,2次/d。治疗组在对照组的基础上口服孕康糖浆,20 m L/次,3次/d。两组患者连续治疗12周。比较两组治疗前后临床疗效和血清细胞因子水平差异。结果治疗后,对照组总有效率和妊娠成功率分别为80.85%和74.47%,均分别显著低于治疗组的95.74%和93.62%,两组比较差异均具有统计学意义(P0.05)。治疗后,两组患者血清肿瘤坏死因子-α(TNF-α)、血管内皮生长因子(VEGF)和干扰素-γ(IFN-γ)水平均显著降低,而诱骗受体3(Dc R3)和转化生长因子-β_1(TGF-β_1)水平均明显升高,同组比较差异具有统计学意义(P0.05);且治疗组上述血清细胞因子指标改善水平显著优于对照组,两组比较差异具有统计学意义(P0.05)。结论孕康糖浆联合地屈孕酮治疗复发性流产可有效提高妊娠率和改善机体细胞因子水平,具有一定的临床推广应用价值。

Clinical study on Yunkang Syrup combined with dydrogesterone in treatment of recurrent spontaneous abortion

Objective To explore the clinical efficacy of Yunkang Syrup combined with dydrogesterone in treatment of recurrent spontaneous abortion. Methods Patients (94 cases) with recurrent spontaneous abortion in the Second Affiliated Hospital of Xinjiang Medical University from January 2014 to January 2016 were divided into control (47 cases) and treatment (47 cases) groups based on different treatments. Patients in the control group were po administered with Dydrogesterone Tablets, 10 mg/time, twice daily. Patients in the treatment group were po administered with Yunkang Syrup on the basis of the control group, 20 mL/time, three times daily. Patients in two groups were treated for 12 weeks. After treatment, the difference of clinical efficacy and serum cytokine levels in two groups before and after treatment was compared. Results After treatment, the clinical efficacy and pregnancy success rate in the control group was 80.85% and 74.47%, which were significantly lower than 95.74% and 93.62% in the treatment group, respectively, and there were differences between two groups (P < 0.05). After treatment, the VEGF, TNF-α, and IFN-γ levels in two groups significantly decreased, but DcR3 and TGF-β1 levels increased, and the difference was statistically significant in the same group (P <0.05). And these serum cytokine levels in the treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Yunkang Syrup combined with dydrogesterone can effectively improve pregnancy rate and improve the body cytokine levels in treatment of recurrent spontaneous abortion, which has a certain clinical application value.
