综合疗法治疗下肢淤血性溃疡37例临床观察

摘    要:目的探讨下肢淤血性溃疡的有效治疗方法。方法下肢淤血性溃疡患者37例(共42条患肢),男21例,女16例,按照国际静脉联盟CEAP分级均为C6,采用下肢静脉腔内激光治疗术(EVLT)加筋膜下交通支结扎,术前于溃疡表面取样进行细菌培养和药物敏感实验,术后患肢抬高、加压包扎和湿性敷料局部处理溃疡。结果42条患肢溃疡均在1月内愈合,8条患肢一期愈合,34条患肢瘢痕愈合。结论腔内激光治疗术加筋膜下交通支结扎,辅以术前细菌培养、药物敏感实验,术后患肢抬高、加压包扎及湿性敷料综合治疗下肢淤血性溃疡,疗效可靠,效果优于未手术湿敷换药。

Abstract:Objective To study the effective treatment of stagnant ulcer in the low limb. Methods 37 patients with a total of 42 lower limbs were treated by EVLT and subfascial communication vein ligation, with the bacterial culture and the drug sensitivity assay on the ulcer;raised and pressured dressing the affected limbs and used the moist dressing on the ulcer for local adjustment after the operation. According to CEAP classification,there were C6 in 42 limbs, Results The ulcer of the 42 limbs were healed within a month, first intention in 8 limbs and scar healing in 34 limbs. Conclusion It was shown that the comprehensive treatment was effective for the stagnant ulcer in the low limb. It was mainly that the EVLT and subfascial communication vein ligation. And we did the bacterial culture and the drug sensitivity assay before the operation, raised and pressured dressing the affected limbs, used the moist dressing on the ulcer after the operation. It was better than only giving moist dressings without operation.
