肾动脉阻力指数和搏动指数对移植肾功能的预测作用

摘    要:目的运用彩色多普勒超声早期测量肾动脉和叶间动脉的阻力指数和搏动指数,观察两者能否预测无明显并发症同种异体肾移植患者术后1年的肾功能。方法对100例无并发症肾移植患者,应用彩色多普勒超声,于肾移植术后1周内,测量移植肾肾动脉和叶间动脉的阻力指数和搏动指数,并与术后1年的移植肾肾小球滤过率(GFR)进行相关分析。结果术后1周测得的肾动脉的阻力指数和搏动指数与术后1年的GFR无明显的相关关系;叶间动脉的阻力指数和搏动指数与术后1年的GFR呈明显的负相关,叶间动脉阻力指数和(或)搏动指数升高,移植后1年肾功能下降,如果叶间动脉阻力指数≥0.7或者搏动指数≥1.2,移植后1年肾功能有明显的下降。结论早期多普勒彩超测量叶间动脉的阻力指数和搏动指数能够预测无明显并发症的移植肾的术后1年肾功能。

Abstract:Objective To study resistance index(RI)and pulsatile index(PI)of main renal artery and interlobar artery whether they can predict 1year allograft function in renal transplantation recipients without distinct complication.Method RI and PI of main renal artery and interlobar artery were obtained by color Doppler ultrasonography within the first week of transplantation,then they were analysed in the correlation with allograft glumerular filtration rate(GFR)at 1year.Results There was no correlation between RI and PI of main renal artery and GFR at 1year;there was a negative correlation between RI and PI of interlobar artery and GFR at 1year.Patients with impaired allograft function had higher RI and PI values.There was a significant decline in allograft function among cases with either RI ≥0.7 or PI ≥1.2.Conclusion RI and PI of interlobar artery measured by Doppler ultrasonography can be used to predict the postoperative 1-year outcome of renal transplantation recipients without distinct complication.
