不规则抗体检查在临床输血中的应用

目的 :检查有输血史或妊娠史的患者血清 (浆 )中不规则抗体 ,以减少或杜绝溶血性输血反应的发生。方法 :用polybrene对 35 15例标本进行不规则抗体筛选 ,用酶、抗球蛋白法进行鉴定 ,并观察不规则抗体检出的阳性率。结果 :不规则抗体筛选阳性 13例 ,阳性率为 0 .37%,通过鉴定实验 ,其中 ,抗 D 9例 ,抗 E 2例 ,抗 A11例 ,抗 M 1例。患者输入无相应抗原的红细胞后 ,血红蛋白值平均上升 2 0g/L (P <0 .0 5 )。结论 :不规则抗体检查 ,能有效地降低或避免溶血性输血反应的发生 ,保证输血的安全性。

Detection of irregular antibodies in clinical blood transfusion

Objective:To detect irregular antibodies in the serum samples of patients who had a history of blood transfusion or pregnancy,in order to decrease or avoid hemolytic transfusion reaction.Method:3515 samples were screened with polybrene and identified with enzyme and antiglobulin test for irregular antibodies,and the positive rate was observed.Result:13 were positive by screening and the positive rate was 0.37%, among them,there were 9 anti-D,2 anti-E,1 anti-A 1 and 1 anti-M.After transfusing erythrocyte of irrelative antigens,the mean valumes of patients, hemoglobin concentration increased by 20 g/L (P< 0.05).Conclusion:Detection of irregular antibodies in patients serum can effeciently decrease or avoid hemolytic transfusion reaction and ensure the safety of blood transfusion.
