声学密度技术在肾功能正常肾病患者中的应用

摘    要:目的:探索声学密度技术在肾功能正常但肾组织活检发现异常的患者中的应用。方法:选取生化检查显示肾功能正常但是肾组织活检发现异常的患者50例,正常人22例(肾脏44只)。将正常对照组及病例组均分成<40岁,40~59岁及≥60岁三个年龄段,测量和分析各组肾皮质、髓质的IBS(Intergratedbackscatter,IBS)、校正的IBS(IBS%)。结果:①<40岁,40~59岁及≥60岁三个年龄段的病例组患者的肾皮质、髓质的IBS%与正常对照组相比均增高且具有统计学意义。②<40岁的病例组的患者与40~59岁及≥60岁组患者比较,其肾皮质、髓质的IBS%较低且具有统计学意义,后两个年龄段之间比较则无统计学差别。结论:①病例组患者的IBS%值已经发生了改变,利用声学密度技术可以早期发现此类患者,利于早期治疗。②年龄对肾脏的IBS%值具有较明显的影响,在分析肾脏的声学密度特点时应引入年龄因素。

Abstract:Objective To evaluate the application of acoustic densitometry(AD) in patients with normal renal function but abnormal biopsy. Method 50 patients whose renal function were normal but biopsy results were abnormal were selected ,the control group was composed of 22 healthy subjects(44 kindeys). All subjects were divided into three groups: less than 40 years(<40y),40~59 years (40~59y) and more than or equal to 60 years(≥60y). AD technology was applied to measure the intergrated backscatter(IBS)and the normalized value of IBS(IBS%) of renal cortex and medulla of different groups. Results ①Cortex and medullar IBS% of patients with normal renal function but abnormal biopsy in three age groups were significantly higher than those of contrasts. ②Among patients with normal renal function but abnormal biopsy, cortex and medullar IBS% in <40y group were significantly lower in comparison with both 40~59y and ≥60y,but no statistical difference was found between 40~59 and ≥60. Conclusion Cortex and medullar IBS%of patients with normal renal function but abnormal biopsy had been different from those of contrasts. With acoustic densitometry, we can differentiate such patients early, and effective treatment can be enduced early. In addtion, it is necessary for physicians to consider the effects of aging factor because age was related closely to IBS%.
