罗哌卡因复合地塞米松的临床实验观察

目的探讨地塞米松与盐酸罗哌卡因配伍用于臂丛阻滞是否可延长镇痛时间并减少不良反应。方法187例臂丛阻滞麻醉下行上肢骨折内固定手术的患者,随机分两组：A组（93例）：0．375％-0．5％盐酸罗哌卡因＋地塞米松（单侧用5mg,双侧10mg）;B组（94例）：单纯用0．375％～0．5％盐酸罗哌卡因,术后记录有效镇痛（VAPS≤3分）维持时间及4、8、16、32、64h各时间段的镇痛有效例数及不良反应发生情况。结果A组术后有效镇痛维持时间平均为20．3h,B组平均为3．5h。结论地塞米松与盐酸罗哌卡因配伍可显著延长罗哌卡因臂丛阻滞的时间,且术后恶心、呕吐的发生显著减少。

Clinical and experimental study on the treatment of ropivacaine combined with dexamethasone

Objective To investigate the compatibleness of dexamethasone and hydrochloric acid ropivacaine used in brachial plexus block whether can be used to extend the analgesia time and to reduce the side effects or not. Methods 187 ca-ses of brachial plexus block patients receive the upper limb fracture fixation surgery after anesthesia. The patients were divided into two groups randomly: Group A (93 cases) :0.375%～0.5% hydrochloric acid ropivacaine +dexamethasone (unilateral use of 5mg, double side 10mg) ; Group B (94 cases) : only use0.375%～0.5% ropivacaine hydrochloride. Record the effective an-algesic time after the surgery ( VAPS≤3 points), and the cases which maintain the effective analgesic after 4,8,16,32,64h and the occurrence of side effects. Results The average time to maintain the effective analgesic of Group A was 20.3h and Group B was 3.5h. Conclusion The compatibleness of dexarnethasone and ropivacaine hydrochloride can extend the time of ropivacaine to block brachial plexus effectively ; and the occurrence of nausea and vomiting decreased significantly after the operation.
