肠易激综合征各亚型患者的生活质量和心理特征分析

目的分析按照罗马Ⅲ标准诊断肠易激综合征(IBS)患者4个亚型之间生活质量和心理特征。方法应用汉化版简明健康调查量表(SF-36)、焦虑自评量表(SAS)和抑郁自评量表(SDS)对腹泻型肠易激综合征(IBS-D)、便秘型肠易激综合征(IBS-C)、混合型肠易激综合征(IBS-M)、不定型肠易激综合征(IBS-U)和正常对照者进行评分,分析各组之间的差异。结果4个亚型之间性别构成比较差异有显著性,女性IBS-C型和IBS-M型患者较多;4个亚型患者SAS和SDS得分比对照组均明显升高,女性患者抑郁得分明显高于男性;4个亚型生理职能、躯体疼痛、一般健康状况、社会功能、情感职能、精神健康等各个维度分值均有明显降低。结论各亚型IBS患者生活质量均受到影响,不良精神心理因素可能是其生活质量主要影响因素。

An analysis of quality of life and mental characteristics in patients with various subtype irritable bowel syndrome

Objective ble bowel syndrome （IBS） To analyze quality of life and mental characteristics in patients with 4 subtypes of irritadiagnosed according to Roman HI criterea. Methods Chinese short form 36 health surey questionnaire （SF- 36）, selfrating anxiety scale （SAS） and self-rating depression scale （SDS） were used to evaluate and analyze differences among patients with IBS of diarrhea type （IBS - D）, with IBS of constipated type （IBS-C）, with IBS of mixed type （IBS-M）, with IBS of undetermined type （IBS-U）, and normal controls. Results A component ratio of sex among 4 subtypes of IBS differed significantly, more females suffered with IBSC and IBS- M. Patients with 4 subtypes of IBS had higher scores of SAS and SDS, compared to the control group. Female patients had higher scores of depression, compared to male patients. The scores of physiological function, health status, social function, feeling function and psychiatric health decreased among 4 subtypes of IBS. Conclusion Every subtype of IBS impacts on patients＇ quality of life, unhealthy psychiatric and psychological factors were main factors to affect their quality of life.
