摘    要:目的:探讨早期不同剂量氨基酸在早产儿中的应用效果。方法整群选择该院2014年12月—2015年12月出生体重在1000~2000 g的早产儿150例,将其平均分为研究组与对照组。研究组在出生24 h内输入高剂量氨基酸(2 g/kg以上,每天递增1 g/kg,直至3.6 g/kg),对照组出生24 h内输入低剂量氨基酸(1 g/kg,每天递增0.5 g/kg,直至3.6 g/kg)。观察比较两组患儿干预前与入院1周后血尿素氮、前白蛋白、血浆剩余碱含量,以及早产儿营养干预的不良反应。结果研究组入院干预1周后血尿素氮(5.4±0.4)mmol/L、前白蛋白(198.5±10.5)g高于对照组(4.4±0.8)mmol/L、(170.5±10.2)g(P<0.05),血浆剩余碱含量(3.0±0.6)mmol/L低于对照组(3.7±0.7)mmol/L(P<0.05)。结论早期高剂量小儿氨基酸在早产生中具有显著的应用效果,可以有效保证生长发育质量,安全可靠,适于临床应用。

Analysis of Application Effect of Early Different Doses of Amino Acid in the Premature Infants
Abstract:Objective To discuss the application effect of early different doses of amino acid in the premature infants. Methods 150 cases of premature infants born in our hospital from December 2014 to December 2015 whose weight were between 1 000 g and 2 000 g were selected and divided into two groups, the research group were injected high-dose amino acid within 24 h after birth (above 2 g/kg, increase 1 g/kg per day, up to 3.6 g/kg), the control group were injected low-dose amino acid within 24 h after birth, (1 g/kg, increase 0.5 g/kg per day, up to 3.6 g/kg), the contents of blood urea nitrogen, prealbumin and base excess of blood before intervention and in 1 week after admission and toxic and side effects of nutri-tional intervention of premature infants of the two groups were observed. Results The blood urea nitrogen and prealbumin in 1 week of intervention after admission in the research group were higher than those in the control group (5.4±0.4) mmol/L, (198.5±10.5) g vs (4.4±0.8) mmol/L, (170.5±10.2) g](P<0.05), and the content of base excess of blood in the research group was lower than that in the control group(3.0±0.6) mmol/L vs (3.7±0.7) mmol/L](P<0.05). Conclusion Early high dose of pe-diatric amino acid has an obvious application effect in the premature infants, and it can effectively guarantee the quality of growth and development, and it is safe and reliable and is suitable for the clinical application.
