宫颈B超测量联合fFN在先兆早产预测中的作用研究

摘    要:目的:研究宫颈长度的B超测量和胎儿纤维连接蛋白(fFN)联合检测在先兆早产预测中的作用。方法整群选取2014年3月15日—2015年3月15日在该院住院保胎治疗的96例先兆早产病例作为研究对象。根据检测方法不同,设置分组。A组:超声检测组,B组:fFN检测组,C组:B超测量联合fFN组。96例病例均采用阴道B超测量宫颈长度,同时进行胎儿纤维连接蛋白测定,追踪并记录其妊娠结局。结果 A组宫颈长度预测先兆早产的敏感度、特异度、阳性预测值和阴性预测值分别是92.86%、57.4%、62.90%、91.18%。 B组fFN预测先兆早产的敏感度、特异度、阳性预测值和阴性预测值分别是88.10%、64.8%、66.07、87.50%。 C组宫颈B超测量联合fFN预测先兆早产的敏感度、特异度、阳性预测值和阴性预测值分别是83.3%、94.44%、92.11%、87.93%。结论 C组B超测量宫颈长度联合胎儿纤维蛋白,可提高先兆早产预测的特异度和阳性预测值,对于先兆早产的预测具有一定的意义。

Research on Effect of Cervical B-Ultrasound Measurement Combined with fFN in Predicting the Threatened Premature Labor
Abstract:Objective To research the effect of the combined detection of B-ultrasound measurement of cervical length and fetal fibronectin in predicting the threatened premature labor. Methods 96 cases of patients with threatened premature labor receiving tocolytic therapy in our hospital from March 15, 2014 to March 15, 2015 were selected as the research object and divided into the groupA (ultrasonic detection group), the group B (fFN detection group) and the group C (B-ultrasound mea-surement combined with fFN group) according to the different detection methods, the cervical lengths of all cases were mea-sured by vaginal B-ultrasound, at the same time, the fFN was measured, and the pregnancy outcomes were traced and recorded. Results The sensitivity, specificity, positive predictive value and negative predictive value were respectively 92.86%, 57.4%, 62.90% and 91.18% in the group A, 88.10%, 64.8%, 66.07% and 87.50% in the group B and 83.3%, 94.44%, 92.11%and 87.93%in the group C. Conclusion B-ultrasound measurement of cervical length and fetal fibronectin can improve the specificity and positive predictive value of in the prediction of threatened premature labor, and it is of a certain significance to the prediction of threatened premature labor.
