乙肝患者血清前S1抗原检测及其临床价值

目的：探讨和分析血清前S1抗原(Pres1）在乙肝感染患者临床检测中的检测方法和应用价值。方法方便选取该院2014年3月—2015年2月所收治的乙型肝炎感染患者190例作为研究对象，通过酶联免疫吸附试验方法，检测患者的血清前S1抗原和乙肝病毒5项指标，并运用荧光定量PCR法，来检测患者的HBV-DNA指标，然后分析血清前S1抗原和HBV-DNA和HBeAg指标的阳性。结果经检测，以HBV-DNA阳性为标准，患者血清前S1抗原和HBV-DNA检测的阳性率分别为96.9%和98.5%，患者的该两项指标对比，差异无统计学意义(χ2=0.037，P>0.05)；以血清 HBsAg指标阳性结果为标准，患者血清前S1抗原和HBeAg指标检测的阳性率分别为88.5%和96.6%，患者的该两项指标对比，差异无统计学意义(χ2=1.362，P>0.05)。结论乙肝病毒前S1抗原可以较好地反映出HBV的复制状况，对于乙肝感染者开展血清前S1抗原检测，能够明显地降低单纯血清学检验中的假阴性率，并降低误诊和漏诊的发生率，值得临床推广。

Detection of Serum Pre-s1 Antigen of Hepatitis B Patients and Its Clinical Value

Objective To discuss and analyze the detection method and application value of serum pre-s1 antigen in the clinical detection of hepatitis B patients. Methods 190 cases of hepatitis B patients admitted and treated in our hospital from March 2014 to February 2015 were selected as the research objects, the serum pre-s1 antigen and 5 indexes of hep-atitis B virus of patients were detected by the enzyme-linked immunosorbent assay method, the HBV-DNA index of pa-tients was detected by the fluorescence quantitative PCR method, and then the positive rates of serum pre-s1 antigen, HBV-DNA and HBeAg indexes and correlation between them were analyzed. Results The positive rates of serum pre-s1 antigen and HBV-DNA detection were respectively 96.9% and 98.5% taking the HBV-DNA positive results as standards, and there was no obvious difference by comparison and there was no statistical significance, (χ2=0.037, P>0.05);the positive rates of serum pre-s1 antigen and HBeAg index detection of patients were respectively 88.5% and 96.6% taking the serum HBsAg index positive results as standards, and there was also no obvious difference by comparison, and there was no statis-tical significance,(χ2=1.362, P>0.05). Conclusion Hepatitis B virus pre-s1 antigen can better reflect the replication status of HBV, and it can obviously decrease the false negative rate of simple serologic testing and decrease the incidence rates of misdiagnosis and missed diagnosis to carry out the detection of serum pre-s1 antigen for patients with hepatitis B infection, which is worth clinical promotion.
