维持性血液透析患者成纤维细胞生长因子23与心脏结构和功能的关系

摘    要:目的:探讨维持性血液透析(MHD)患者成纤维细胞生长因子23(FGF23)与心脏结构和功能的关系。方法选取该院2014年1—12月收治的180例采用MHD治疗的患者作为研究对象,对所有患者进行血清FGF23水平检测,并采用四分位法按血清FGF23水平将患者分成4组,每组45例。对患者进行超声心动图检查,检测患者的心脏结构和功能,并分析血清FGF23水平和心脏结构和功能的相关性。结果对照组MPI、LVEF、LVMI水平及LVH发生率分别为(23.72±3.66)%、(75.25±5.83)%、(89.42±12.89)g/m2、0%,MHD患者的MPI、LVMI水平以及LVH发生率均明显高于对照组,LVEF水平明显低于对照组,且随着MHD患者血清FGF23水平的逐渐上升,MPI、LVMI水平及LVH发生率随之逐渐上升,LVEF逐渐下降(P<0.01);MHD患者血清FGF23与MPI、LVMI、LVH呈正相关(r=0.406~0.706,P<0.01),与LVEF呈负相关(r=-0.586,P<0.01)。结论 MHD患者血清FGF23水平明显上升,血清FGF23水平与心脏结构和功能存在密切相关性。

Correlation between Fibroblast Growth Factor 23 in Patients with Mainte-nance Hemodialysis and Cardiac Structure and Function
Abstract:Objective To study the correlation between fibroblast growth factor 23 in patients with maintenance hemodialy-sis and cardiac structure and function. Methods 180 cases of patients receiving MHD treatment admitted and treated in our hospital from January 2014 to December 2014 were selected as the research objects and the serum FGF23 levels of all pa-tients were detected, and the patients were divided into four groups with 45 cases in each according to the serum FGF23 level by quartile method, and the cardiac structures and functions of patients were detected by the echocardiography exami-nation, and the correlation between the serum FGF23 level and cardiac structure and function was analyzed. Results The incidence rates of MPI, LVEF and LVMI levels and LVH in the control group were respectively (23.72±3.66)%, (75.25± 5.83)%, (89.42±12.89) g/m2 and 0%,and the incidence rates of MPI and LVMI levels and LVH in the MHD patients were obviously higher than those in the control group, and the LVEF level was obviously lower than that in the control group, and with the gradual increase of the serum FGF23 level of MHD patients, the incidence rates of MPI, LVM levels and LVH gradually increased, but LVEF gradually decreased(P<0.01);and the serum FGF23 of MHD patients was positively correlat-ed with the MPI, LVMI and LVH (r=0.406~0.706, P<0.01), but negatively correlated with LVEF(r=-0.586, P<0.01). Con-clusion The serum FGF23 level of MHD patients obviously increases, and the serum FGF23 level is closely correlated with the cardiac structure and function.
