摘    要:目的 探讨食管癌的CT表现及诊断价值.方法 对42例经手术病理或胃镜活检证实为食管癌患者的手术病理资料及CT表现进行回顾性分析.结果 42例食管癌患者均有食管壁增厚,其中食管壁环形增厚35例,管壁偏侧性增厚7例.邻近组织浸润17例,其中肿瘤侵犯气管7例、血管6例、纵隔胸膜3例、心包1例,其中3例是多结构同时侵犯.纵膈淋巴结肿大转移14例,肝转移3例,肺转移2例.结论 CT能显示食管癌的形态、大小,判定有无周围浸润、淋巴结及远处脏器转移,对疾病的评估和治疗方案的选择具有重要的价值.

Abstract:Objective To investigate the diagnostic value of CT findings in esophageal carcinoma. Methods The surgical and pathological data and CT findings were retrospectively analyzed in 42 patients with esophageal carcinoma confirmed by surgical pathology or gastroscope biopsy. Results All the 42 patients had thickened esophageal walls. Among them, circular thickening was found in 35 and lateral thickening in 7. Seventeen patients had adjacent tissue infiltration. Among them, tracheal invasion occurred in 7, blood vessel invasion in 6, chest membrane invasion in 3, pericardium invasion in 1, and multi-tissue invasion in 3. In addition, enlarged mediastinal lymph node metastasis was observed in 14 of the 42 patients, liver metastases in 3 and lung metastasis in 2. Conclusion CT scanning can display the shape and size of esophageal carcinoma and demonstrate surrounding infiltration and lymph node and distant organ metastasis. Therefore, it has important value for disease assessment and treatment option.
