一侧附件异常回声区对宫内早期妊娠的诊断价值

目的探讨一侧附件异常回声区对宫内早期妊娠的诊断价值。方法对90例早期妊娠患者双侧附件的经阴道超声表现进行回顾性分析。结果 63例(70%)在一侧卵巢内见到一异常回声区,大小10 mm×16 mm～20 mm×17 mm,左侧55例,右侧8例,其形态规则,呈均匀中等回声区,其中17例中间可见不规则暗区;彩色多普勒血流显像(CDFI):异常回声区内部及周边血流丰富,阻力指数(RI)均小于0.5。其余27例(30%)无异常发现。结论当尿HCG阳性,经阴道超声检查未见宫内孕囊或类孕囊结构,但发现一侧卵巢内有异常回声区,且CDFI显示异常回声区内探及丰富血流,RI小于0.5时,仍应考虑为宫内早孕可能性。

Diagnostic Value of Abnormal Echo Area in Unilateral Adnexa in Early Intrauterine Pregnancy

Objective To explore the diagnostic value of abnormal echo area in unilateral adnexa in early intrauterine pregnancy. Methods Transvaginal ultrasound findings of bilateral adnexa of 90 patients with early pregnancy were analyzed retrospectively. Results Abnormal echo area （10 mm× 16 mm-20 mm×17 mm）was found in unilateral adnexa in 63 （70%） of the 90 cases （left adnexa, 55 cases; right adnexa, 8 cases）, and the uniform and medium echo area showed regular shape. Among the 63 cases, irregular dark area was found in 17. Color Doppler flow imaging （CDFI） showed rich blood flow in and around the abnormal echo area （RI〈0.5）. No abnormal echo was found in 27 （30%） of the 90 eases. Conclusion Early intrauterine pregnancy should be considered when transvaginal ultrasound showed abnormal echo area in unilateral adnexa without in the uterus and CDFI showed rich blood flow in the positive patients. gestational sac or gestational sac-like structure abnormal echo area （RI〈0.5） in urine HCG
