双侧子宫动脉栓塞治疗子宫多发肌瘤的临床应用

目的 探讨双侧子宫动脉栓塞术(UAE)治疗子宫多发肌瘤患者的临床应用价值.方法 对36例子宫多发肌瘤患者均行双侧UAE治疗.术后3个月复查MRI,对手术前及术后3个月肌瘤大小及血供的变化情况进行比较.结果 术后3个月MRI检查示,子宫肌瘤最大径(6.24±0.78)cm较术前(8.36±0.92)cm明显缩小(P＜0.05).子宫肌瘤血液供应减少,69.4％(25/36)患者瘤体内部血流消失.25.0％(6/24)的患者月经量减少,经期较术前规律,75.0％(18/24)的患者月经恢复正常,87.5％(14/16)的患者下腹部坠胀症状缓解,58.3％(7/12)的患者尿频尿急症状好转,60.0％(6/10)的患者贫血症状改善.结论 行双侧UAE治疗子宫多发肌瘤具有良好的临床治疗效果.

Clinical Application of Bilateral Uterine Artery Embolization in Multiple Uterine Fibroids

Objective To evaluate the clinical value of bilateral uterine artery embolization （UAE） in the treatment of multiple uterine fibroids. Methods Thirty-six patients with multiple uterine fibroids were treated with bilateral UAE and were re-examined with MRI scan 3 months after operation. The size and blood supply of fibroids and blood supply were measured before and 3 months after operation. Results The maximum diameter of uterine fibroids at 3 months after operation was smaller than that before operation [ （6.24±0.78） vs （8.36±0.92）cm, P〈0.05]. Furthermore, the biood supply of uterine fibroids decreased in all patients and tumor blood flow disappeared in 25 patients （60.4%）. Among the 24 patients with menstrual flow increase and menstrual cycle disorder, menstrual flow decreased in 6 （25.0%）and returned to normal in 18 （75.0%）. Among the 16 patients with lower abdominal bulge, symptoms were relieved in 14 （87.5%）. Among the 12 patients with urinary urgency, symptoms were relieved in 7 （58.3%）. Among the 10 patients with anaemia, symptoms were relieved in 6 （60.0%）. Conclusion Bilateral UAE is effective for multiple uterine fibroids.
