急性视神经炎的临床分析

目的：观察和分析急性视神经炎的发病原因，治疗效果，和复发情况。方法：对79例128眼急性视神经炎的视力、发病原因、FFA、视野、瞳孔运动障碍和P－VEP的改变，治疗效果和预后进行综合分析。结果：急性视神经炎视功能损害重，发病原因多，以双眼发病多见。视野可出现多种改变，以中心暗点多见；瞳孔运动障碍者60．16％，表现为黑朦性瞳孔强直，弱视瞳孔无力和Marcus Gunn瞳孔阳性，以黑朦性瞳孔强直视功能损害重。P－VEP异常率高，异常程度和视功能损害平行。急性视神经炎治愈率59．28％，有效率85．94％，以中毒性视神经炎治疗效果最差。脱髓鞘疾病引起的易复发。结论：急性视神经炎视功能损害重，治疗效果较好。瞳孔改变和P－VEP对本病的诊断和疗效判断和估计预后有一定的临床价值。

The clinical analysis of acute optic neuritis

Objective To observe and analyse the causes,the curative effect and the recurrence in AON.Methods 79 cases(128 eyes)of AON were analysed in visual acuity,causes,FAA,visual field,disturbance of pupil movement,VEP,curative effect and prognosis.Results The severe damage of visual function,more causes and bilateral involving were found in AON.Visual field has different manifestations,central scotomas were the most common Disturbance of pupil movement was 60 16%,which showed amaurotic pupil rigidity(APR),amblyopic pupillary inertia and positive of Marsu Gunn pupil,the visual function of APR was damaged heavily.Abnomal rate of VEP was higher,the degree of abnormality paralleled the damage ofvisual function.The cure rate of AON was 59 38%,the effective rate was 85 85%,among them,the curative effect of toxic neuritis was the most bad,demyelimating disease was easily recurrent.Conclusions Visual function of AON was damaged heavily,but cuative effect was well.The chang of pupil and VEP can be helpful in diagnosis,judging curative effect and prognosis.
