子宫颈环形电切术在宫颈上皮内瘤样病变诊治中的应用简

目的：探讨子宫颈环形电切术（ Loop electrosurgical excision procedure ,LEEP）在宫颈上皮内瘤样病变（ Cervical intraepithelial neoplasia ,CIN）诊治中的应用。方法对72例阴道镜下活检为CINⅠ～Ⅲ级及36例可疑CIN患者进行LEEP术治疗,随访观察各组临床效果及并发症发生情况。结果108例患者术后出血量为（22．5±4．5）ml,创面愈合时间为（6．8±2．3）周;CINⅠ～Ⅲ级组、可疑CIN组治愈率分别为95．83％、94．44％,两组治愈率比较差异无统计学意义（P＞0．05）;术后并发症发生率为20．37％;术后病理学检查与术前诊断符合率为91．67％。结论LEEP术适用于CIN及宫颈细胞学与阴道镜检查不一致、无法排除浸润癌、病理学检查异常的可疑CIN患者的治疗,手术时间短、出血量少,术后并发症少。

Clinical applications of cervical loop electrosurgical excision procedure in the diagnosis and treatment of cervical intraepithelial neoplasia

Objective To understand loop electrosurgical excision procedure in the diagnosis and treatment of cervical intraepithelial neoplasia .Method 72 cases of coolposcopy and biopsy as CIN Ⅰ ~III grade CIN and 36 patients suspected of CIN were treated with LEEP and observed for clinical effects and complications .Findings Postoperative blood loss was （22.5 ±4.5） ml;the wound healing time was （6.8 ±2.3） weeks;cure rates of the CINⅠ~Ⅲlevel group and the group suspicious of CIN was 95.83%and 94.44%respectively.The difference of cure rate between the two groups was not statistically significant （P〉0.05）;postoperative complication rate was 20.37%.Conclusion LEEP surgery can be an effective treatment for CIN and cervical cytology where colposcopy is inconsistent , invasive can not be excluded , and pathology abnormalities are found of patients suspected of CIN .It ensures shorter operative time , less blood loss, and fewer complications.
