儿童系统性红斑狼疮与类风湿性关节炎患者血细胞黏附能力与红细胞CD35分子比较研究

摘    要:目的:研究儿童患者中类风湿性关节炎(rheumatoid arthritis,RA)和系统性红斑狼疮(systemic lulups erythematosus,SLE)血细胞的天然免疫功能状况,比较在儿童患者中这两种疾病红细胞表面CD35定量活性变化,探讨两种疾病的发病机制。方法:分离新鲜红细胞、淋巴细胞、粒细胞三系,分别测定其对肿瘤细胞的快速天然免疫黏附反应。应用流式细胞仪测定红细胞表面CD35的表达水平。结果:RA儿童患者红细胞、淋巴细胞以及粒细胞对肿瘤细胞黏附率均明显高于正常对照组(P<0.001);SLE儿童患者血细胞对肿瘤细胞黏附率又明显低于正常对照组(P<0.001);RA儿童患者的血细胞对肿瘤细胞黏附率,进展期最高,静止期其次,消退期再次,RA儿童患者各期均明显高于正常对照组(P<0.05)。RA儿童患者红细胞CD35明显高于正常对照组和SLE患者组(P<0.001);而SLE儿童患者CD35明显低于正常对照组(P<0.001);类风湿性关节炎红细胞CD35分子与红细胞黏附率呈显著正相关(γ=0.804,P<0.05)。结论:儿童SLE和RA患者红细胞天然免疫功能存在紊乱,并可能与病情的活动性有关。红细胞免疫功能检测可作为判定儿童患者RA与SLE病情演变及预后的指标之一。
