摘    要:目的 建立一种早期、快速诊断人巨细胞病毒(human cytomegalovirus,HCMV)感染的抗原检测方法,以了解肾移植术后受者的HCMV感染情况并探讨其临床意义。方法 利用抗HCMV前早期抗原和早期抗原的单克隆抗体,建立了免疫组化Envision^TM二步法。用于检测外周血多形核白细胞(PMNL)中HCMV抗原(前早期抗原和早期抗原),诊断HCMV活动性感染。结果 检测肾移植术后受者86

Abstract:Objective To provide a rapid and early antigen diagnostic technique for the patients with human cytomegalovirus(HCMV) infection after renal transplantation and discuss its clinical significance. Methods A two-step tech-nique of immunohistochemical EnvisionTM was developed to detect antigen in peripheral blood polymorphonuclear leucocytes(PMNL) using HCMV monoclonal antibody of immediate early antigen(IEA) and early antigen (EA) to diagnose active infection of HCMV. Results This assay was not disturbed by background and nonspecificity of stain. It is easy to distinguish positive cells from negative cells. HCMV antigen positive cell were counted in every 5. 0× 104 PMNL. Among 86 renal transplan-tation recipients, 39 were positive for HCMV antigen, the positive rate was 45.3%, the average count of positive cells were16. 5/5. 0×104 PMNL. In patients suffering form the HCMV disease and the subclinical infection the average counts of posi-tive cells were 24.5±18.2/5. 0×104 PMNL and 12.0±10. 6/5. 0× 104 PMNL respectively. 15 healthy people (anti-HCMVantibody negative) were also tested with the antigen as control. The results of HCMV antigen were negative. ConclusionThis method is rapid, specific, accurate and stable for detecting HCMV antigen. The latent infection and active infection ofHCMV can be distinguished by this method. It is useful in the early diagnosis of HCMV infection and may be sued as the prin-cipal method for guiding the immunological rejection of therapy and judging prognosis.
