PK阴式与腹腔镜全子宫切除术的临床比较

摘    要:目的比较PK系统开腹钳(PK)阴式与腹腔镜全子宫切除术的临床效果。方法选择100例子宫肌瘤患者,随机分成2组,每组50例,50例借助PK行阴式全子宫切除术(阴式组),50例行腹腔镜全子宫切除术(腹腔镜组),对2组的手术时间、术中出血、单个肌瘤最大径、留置导尿时间、肛门排气时间、住院时间、术后病率、腹部瘢痕、住院费用等进行对照分析。结果两组手术时间、腹部瘢痕及住院费用比较,差异有统计学意义(P〈0.05);而术中出血、单个肌瘤最大径、留置导尿时间、肛门排气时间、住院时间、术后病率比较,差异均无统计学意义(P〉0.05)。结论在同样的技术条件和同样的手术适应证下,借助PK经阴道全子宫切除术是最佳的手术途径。

Abstract:Objective To compare the clinical efficacy of vaginal hysterectomy with PKTM and total laparoscopic hysterectomy. Methods One hundred patients with hysteromyoma were randomly divided into vaginal group ( receiving vaginal hysterectomy with PKTM ) and laparoscopic group ( receiving total laparoscopic hysterectomy ) with 50 cases in each. The operation time, blood loss, maximum diameter of single myoma, time of detain urethral catheterication and passage of gas by anus, days of hospitalization, abdominal scar and cost of hospitalization were compared between the two groups, Results There were significant differences on the operation time, abdominal scar and cost of hospitalization between the two kinds of operation ( P 〈 0,05 ), but not on the blood loss, maximum diameter of single myoma, time of detain urethral catheterication and passage of gas by anus, days of hospitalization ( P 〉 0.05 ). Conclusion Vaginal hysterectomy with PKTM is the optimal operative way on the same technical conditions and operative indications.
