早孕期阴道出血与早产关系的临床研究

目的探讨早期怀孕的阴道出血与早产的关联性。方法分析在我院分娩的孕产妇，评估正常孕妇和早产病例的分布差异，分析阴道出血与早产间的关联性。结果全部病例中早产者295例，孕期阴道出血占25．01％。正常分娩组与早产组比较，生育史和孕期是否工作等无统计学意义（P〉0．05）；孕妇年龄、体重指数和阴道出血等有统计学意义（P〉0．05）。阴道出血的早产危险性为无阴道出血的1．06～1-81倍。早孕期阴道出血与自发性早产的关联性最高（P=0．002）。阴道出血者患轻型早产的危险性为无阴道出血者的1．11～1．97倍（P=0．008）。结论早孕期阴道出血早产风险明显增高，特别是自发性早产及孕周为34～37周早产比例明显增高。

Clinical Study on the Correlation between Vaginal Bleeding during Early Pregnancy and Preterm Delivery

Objective To explore the correlation between vaginal bleeding during early pregnancy and preterm delivery. Methods The differences of distribution were evaluated between normal pregnant and premature delivery patients based on pregnant patients. The correlation between vaginal bleeding during early pregnancy and preterm delivery was investigated based on the stratified analysis. Results There were 295 subjects with preterm delivery and the ratio of vaginal bleeding during pregnancy was 25.01%. There were no statistical differences of reproductive history and work during pregnancy between groups of normal subjects and preterm delivery （ P 〉0.05 ）. There were statistical differences of maternal age, body index and vaginal bleeding in groups （ P 〈 0.05 ）. Compared to subjects without bleeding, the vaginal bleeding in early pregnancy was associated with a 1.06 to 1.81 fold increased risk of preterm delivery. The correlationbetween vaginal bleeding in early pregnancy and spontaneous preterm labor was highest （ P=0.002 ）. Compared to subjects without bleeding, the vaginal bleeding in early pregnancy was associated with a 1.11 to 1.97 fold increased risk of mild preterm delivery （ P=0.008 ）. Conclusion The vaginal bleeding in early pregnancy is associated with an increased risk of preterm delivery. Especially, the ratios of spontaneous preterm labor and mild preterm delivery rise up.
