基础麻醉复合肋间神经阻滞麻醉对呼吸循环功能的影响

目的探讨基础麻醉复合肋问神经阻滞麻醉用于乳腺区段切除手术的镇痛效果和对呼吸循环功能的影响。方法选取ASAⅠ～Ⅱ级患者，年龄20—65岁，拟行乳腺区段切除术。将患者随机分为三组（n=30），A组：静吸复合麻醉；B组：基础麻醉＋局部麻醉；C组：基础麻醉复合0．25％罗哌卡因行腋中线T2～7肋间神经阻滞麻醉。结果综合镇痛质量，术中：A组与C组镇痛效果优于B组（P〈0．05）；术后：B组与C组镇痛效果优于A组（P〈0．05）。C组的平均动脉压、心率波动明显小于A、B组（P〈0．05）。结论基础麻醉复合肋间神经阻滞麻醉用于乳腺区段切除术中与术后均能取得良好的镇痛效果，对呼吸循环功能的影响较小，安全性高。

Clinical research the influence on breath and circulation of basal narcosis plus intercostal nerve block used in operations of mammary gland

Objective To observe the analgesic efficacy of basal narcosis plus intercostal nerve block on breath and circulation in mammary gland operations.Methods Patients were randomly divided into three groups with 30 cases each.Patients in group A were given general anesthesia,in group B were given subcutaneous infiltration; in group C T_(2～7) intercostal nerves were blocked through midaxillary line and 0.25% ropivacaine was used,and midazolan basal narcosis was given in group B and C.Record the analgesia effect and the Vital signs.Results During operation,anaesthesia effect was better in group A and C than in group B(P＜0.05) ; The analgesic efficacy on breath and circulation in group C was lass than group A and B.Conclusion During operation and after operation in mammary gland operations it can get a satified analgesia effect if intercostal nerve block through midaxillary line is taken,and it has less effect on breath and circulation and it is safe.
