硬化剂注射治疗食管曲张静脉模型的病理变化

目的 研究3种不同硬化剂的致硬化作用和其局部毒性.方法 36只新西兰兔的肝前性门脉高压食管静脉曲张模型,随机地分为5%鱼肝油酸钠组、1%乙氧硬化醇组和消痔灵注射液组,每组12只.经腹给兔行硬化剂注射治疗,分别于注射后72h、10天、20天将兔处死,肉眼观察管溃疡发生率并行组织学检查.结果5%鱼肝油酸钠组、1%乙氧硬化醇组的溃疡发生率分别为83.3%和58.3%,组织学上以“液化性坏死”为特征;消痔灵注射液组的溃疡发生率仅为16.7%,组织学上以“凝固性坏死”为特征.凝固性坏死可不经液化而为肉芽组织逐渐包裹、吸收,产生理想的纤维化.结论 消痔灵可能成为高效、低并发症的理想硬化剂.

Pathological changes after injection sclerotherapy with esophageal varices model

Objective To obseve sclerogenesis and local toxicity of three different sclerosants. Methods Thirty-six rabbits with pre-hepatic portal hypertension and esophageal varices were randomized to 5% Sodium Morrhuate, 1% Athoxysclerol.and Xiao-zhiling group,each with 12 rabbits. Sclerotherapy with the three different sclerosant were carried out through abdomenal operation. The rabbits were sacrificed for examination of esophageal ulcer rate and histological changes 72h,10days,and 20 days after the injection. Results In both 5% Sodium Mor-rhuate and 1% Athoxysclerol group,higher esophageal ulcer rate were noted,and histological feature were"liquefaction necrosis"by granulation tissue repair. While in Xiao-zhiling group,lower esophageal ulcer rate was found and histological characteristic was "coagulation necrosis". Co-agulation necrosis could be encapsulated by granulation tissue with organization and fibrosis gradually. Conclusion Xiao-zhiling may become a high efficacy,low complication sclerosant.
