老年人食管癌贲门癌患者手术治疗的临床评估(附62例报告)

目的：评估老年人食管癌、贲门癌患者的手术治疗及手术效果。方法：将62例年龄≥70岁（老年组）和62例年龄＜70岁（非老年组）已行食管癌、贲门癌切除手术患者的临床特征、术后并发症、术后早期死亡率及近、远期临床随访结果进行回顾性对照分析。结果：术后并发症发生率：老年组29．03％（18／62），非老年组32．26％（20／62）；术后早期死亡率：老年组3．23％（2／62），非老年组1．6％（1／62）；近、远期效果：老年组1年生存率74．19％（46／62），2年生存率54．84％（34／62），3年生存率38．71％（24／62）；非老年组1年生存率80．64％（50／62），2年生存率67．74％（42／62），3年生存率32．26％（20／62）。结论：经严格选择病人，恰当及时的术前术后处理，老年组与非老年组术后并发症发生率、早期死亡率及近、无期效果无明显差异。对70岁以上食管癌、贲门癌患者积极行手术治疗是可行的。

EFFECTS OF SURGERY IN TREATING CARDIAC AND ESOPHAGEAL CANCERS IN SENIOR PEOPLE (WITH A REPORT OF 62 CASES)

Objective To evaluate the curative effects of surgery in treating cardiac and esophageal cancers in senior people.Methods 124 patient with cardiac and esophageal cancers were divided into two groups,the senile group including 62 cases at or above seventy and the non senile group including 62 cases under seventy.The clinical characteristics,postoperative complications,early death rate after surgery and the results of short term and long term follow up surveys were reviewed retrospectively.Results The occurrence rates of complication in the senile group and the non senile group were 29.03%(18/62) and 32.26%(20/62) respectively.The rates of early death were 3.23%(2/62) and 1.61%(1/62) respectively.In the senile group,the one year survival rate was 74.19% (46/62),the two year survival rate was 54.84%(34/62) and the three year survival rate was 38.71%(24/62).In the non senile group,the survival rates were 80.64%(50/62),67.74%(42/62) and 32.26%(20/62) respectively.Conclusion There were no significant differences between the two groups in the occurrence rates of postoperative complications,early death after surgery and short term and long term curative effects.Surgery has proven to be feasible in the treatment of cardiac and esophageal cancers in people at or above seventy.
