阻塞性睡眠呼吸暂停综合征持续正压通气的效果观察及护理

目的:观察阻塞性睡眠呼吸暂停综合征(OSAHS)于全身麻醉下悬雍垂腭咽成形术前行持续正压通气治疗(CPAP)的效果,探讨CPAP治疗的护理方法。方法:为45例OSAHS患者行悬雍垂腭咽成形术(UPPP),将术前不做CPAP治疗的24例作为对照组;21例术前行CPAP治疗7d以上,术后第2晚均行多导睡眠监测(PSG),作为实验组。对呼吸暂停低通气指数(AH I)、最低血氧饱和度(LSpO2)以及睡眠结构等指标进行观察。结果:对照组有5例术后第2晚LSpO2比术前下降,AH I升高,余19例LSpO2升高,AH I下降56%。实验组经精心护理,均能良好适应CPAP治疗,治疗后LSpO2升高28%,AH I下降92%。结论:OSAHS患者于UPPP术前采用CPAP治疗能缓解病情,降低手术危险性;有效地护理能增强患者对CPAP治疗的适应性。

Bservation of the effective outcome of continuous positive airway pressure in the treatment of obstructive sleep apnea syndrome and nursing care

Objective: To observe the effectiveness of CPAP before UPPP under general anesthesia in the treatment of obstructive sleep apnea syndrome and explore nursing methods of CPAP.Methods: Forty five patients with obstructive sleep apnea syndrome received UPPP.Twenty four patients,who were not treated with CPAP before UPPP,were taken as control group,and 21 patients,who were treated with CPAP over 7 days and received PSG the second day after UPPP,were taken as experiment group.The indexes regarding AHI,LSpO_2 and sleep composition were analyzed.Results: LSpO_2 descended and AHI rose in 5 patients of control group the second night after UPPP,LSpO_2 rose and AHI descended to 56% in the other 19 patients.The patients of experiment group adapted themselves to CPAP thanks to accurate and careful nursing care,so LSpO2 went up to 28%,AHI came down to 92%.Conclusion: CPAP can effectively improve patient's conditions before UPPP and lessen the risk of operation and increase patient's adaptability to CPAP as well.
