磷酸肌酸对急诊经皮冠状动脉介入治疗术患者左心功能的影响

目的：评价急诊经皮冠状动脉介入治疗（PCI）中应用磷酸肌酸对左心功能的影响。方法：对40例急性前壁心肌梗死患者随机分为对照组（20）与治疗组（20）。治疗组20例在常规治疗的基础上,PCI术前后采用磷酸肌酸注射液静脉滴注,观察术后1h心电图ST段抬高总和回落百分比（sumSTR）、磷酸肌酸激酶同功酶（CK-MB）峰值及左心室射血分数（LVEF）。结果：治疗组患者术后CK-MB峰值低于对照组（P〈0.05）。sumSTR治疗组下降幅度较对照组明显（P〈0.05）。PCI后4周无论治疗组还是对照组LVEF均比3d时有明显改善（P〈0.05）,4周时治疗组LVEF较对照组改善更显著（P〈0.05）。结论：急性心肌梗死患者PCI术围术期静脉应用磷酸肌酸注射液能减轻心肌缺血-再灌注损伤,改善左心功能。

Effects of Creatine Phosphate on Left Ventricular Function in Patients with Acute Myocardial Infarction after Emergency PCI Therapy

Objective：To investigate the effect of creatine phosphate（CP） on left ventricular function in patients with acute myocardial infarction undergoing primary percutaneous coronary intervention （PCI） therapy.Method：40 patients with acute anterior myocardial infarction were randomly divided into treatment group（n=20） and control group （n=20）.Conventional treatments were given to each group,and CP was added to treatment group additionally.Both the resolution of the sum of ST-segment elevation （ST-segment resolution,STR） and the peak value of serum CK-MB were analyzed.Meanwhile,the left ventricular ejection fraction （LVEF） was also measured.Result：The peak value of serum CK-MB in treatment group was lower than that in the control group（P0.05）.1 hour after PCI,the sum STR in treatment group was significantly higher than that in control group （P0.05）.LVEF got better at the fourth week after surgery than at the third day in both groups （P0.05）.Moreover,LVEF in treatment group improved more significantly than control group at the fourth week after PCI（P0.05）.Conclusion：In the perioperative period of PCI,CP can decrease ischemia-reperfusion injury and improve the left ventricular function in AMI.
