新生儿病理性黄疸对机体的损害及其防治研究进展

摘    要:目的探讨新生儿病理性黄疸对心肌和。肾功能的损害,同时关注对其他系统的损害及其研究进展。方法在我科收治的141例新生儿黄疸患儿中,选择病理性新生儿黄疸66例为观察组,并以58例同期正常新生儿为对照组,比较两组新生儿的肌酸激酶(CK)、肌酸激酶同工酶(CK—MB)、肌钙蛋白I(cTnI)的水平与。肾功能血清值BUN、Cr、B2-MG指标。结果观察组与对照组血清总胆红素分别为(281.15±37.88)mmol/L、(129.56±21.77)mmol/L,CK—MB分别为(55.024-26.84)U/L、(16.73±9.80)U/L,差异有极显著性垆〈0.01),肌钙蛋白分别为(0.48±0.64)μg/L、(0.15±0.62)μg/L,差异有显著性(P〈0.05),但所有新生儿均未出现心肌损伤的临床表现,观察组血、尿β2-MG分别为(3.31±0.88)、(1.38±0.61)均高于对照组(2.67±0.53)、(1.09±O.32),差异有显著性垆〈0.01),BUN、血Cr两组间比较差异无显著性(P〉0.05)。结论病理性黄疸对新生儿心肌和肾脏有一定损害,需要引起关注。

Abstract:Objective To explore the damage of pathological jaundice in neonates to the myocardium, kidney function and study its lesion to other systems as well. Methods 66 neonates with pathological jaundice were enrolled into experiment group, while 58 healthy same term neonates as control group. The levels of myocardial enzymes and cTnl and BUN,Cr, 13 2-MG in both groups were compared and analyzed clinically.Results The levels of serum total bilirubin between observation group and control group were (281.15 ± 37.88), (129.56 ± 21.77), CK-MB were (55.02 ± 26.84) and (16.73 ± 9.80) respectively, the difference was statistically significant(P〈 0.01). The blood and urine 13 2-MG in experiment group were (3.31 ± 0.88) and (1.38 ± 0.61), while those in control group were (2.67 ± 0.53) and (1. 09 ± 0.32), the difference was statistically significant(P 〈 0.01). There was no significant difference in BUN and serum Cr levels between the two groups (P 〉 0.05). Conclusion There is obvious damage to myocardium and kidney in the neonates with pathological jaundice.
