腹腔镜术后下肢静脉血栓28例分析

目的:探讨腹腔镜手术后下肢静脉血栓发生的原因和预防措施。方法:回顾性分析我院1999年5月~2006年5月2 000余例腹腔镜手术后出现的28例下肢静脉血栓的临床资料。结果:手术时间20~200m in,15例下肢浅静脉血栓治疗5~7d后症状消失。复查彩色多普勒血栓消失,随访3~12个月,无复发。13例髂—股及股深静脉,经抗凝治疗14~21d症状消失,随访12个月,无复发。结论:腹腔镜术后下肢静脉血栓的发生与患者高危因素、CO2气腹、体位等因素密切相关,在围手术期应积极采取措施加以预防。

Venous thrombosis of the lower limbs following laparoscopic surgery:analysis of 28 cases

Objective:To explore the etiology and prevention of venous thrombosis(VT) of the lower limbs following laparoscopic surgery.Methods:The data of 28 cases of VT following 2000 cases of laparoscopic surgery in our hospital from May 1999 to May 2006 were retrospectively analyzed.Results:The operation time ranged from 20 to 200min.After 15 patients with superficial vein thrombosis were treatal their symptoms vanished.Re-examinations of color Doppler ultrasonography revealed the disappearance of thrombosis.No recurrence was found after 3 months of follow up.In 13 cases of deep femoral vein thrombosis,an anticoagulation therapy was employed for 14-21 days and no recurrence was found after a follow of 12 months.Conclusions:The development of Venous thrombosis(VT) of the lower limbs following laparoscopic surgery is correlated to patients' high-risk factors,carbon dioxide pneumoperitoneum,and position.Preventive measures shoud be adopted actively during periperative period.
