重症肝炎患者医院感染的临床特点和相关危险因素

目的分析重症肝炎患者医院感染(NI)的临床特点和相关危险因素。方法回顾性分析486例重症肝炎患者的临床资料。根据有无发生NI将患者分为感染组和非感染组。比较2组患者相关参数差异。结果重症肝炎患者NI发生部位主要为腹腔感染(40.8%)和呼吸系统(36.8%);主要的致病菌为G+球菌(32.6%)、肺炎克雷伯菌(21.7%)和大肠埃希菌(15.2%)。感染组患者年龄、住院时间、侵入性检查和治疗比例、抗菌药联合和免疫制剂应用比例显著高于非感染组(P<0.05)。结论重症肝炎患者NI的主要感染部位是腹腔感染,G+球菌是其主要的病原菌。重症肝炎患者NI可能与年龄、住院时间、侵入性检查和治疗、抗菌药和免疫制剂应用有关。

High Risk Factors and Clinical Features for Nosocomial Infections in Severe Hepatitis Patients

Objective To investigate the high risk factors and clinical features of nosocomial infections（NI） in severe hepatitis patients.Methods Four hundred and eighty six severe hepatitis patients with NI,were analyzed retrospectively.All patients was divided into NI and non-NI group.The results were compared between the groups.Results The main infection site were the peritoneal cavity（40.8%） and respiratory tract（36.8%）.The main pathogens were positive cocci（32.6%）,Klebsiella pneumoniae（21.7%） and escherichia coli（15.2%）.Age,duration of hospitalization,invasive operation and application of antibiotics were increased significantly in NI group compared to those of non-NI group（P0.05）.There were significantly difference between the two groups in catheter sites,catheter types and sealed tube method（P0.05）.Conclusion The major risk foctors of NI in severe hepatitis patients were age,duration of hospitalization,invasive operation and application of antibiotics.
