临床护士工作压力干预效果评价

目的 通过巴林特小组案例分析,解析临床护士工作中的负性体验,为量性研究奠定基础.方法 分别对两所军队三甲医院具有中等以上工作压力的10名护士各进行6次巴林特小组训练,采用扎根理论的方法对活动记录进行质性分析.结果 两组共呈现临床案例12个,发生情境主题3个:与患者相关的心理冲突、与患者家属的冲突及生命价值的困惑.研究对象的体验:面对“无法治愈的患者”时,体会到生命无常的无助感;与患者或家属发生冲突时,感觉到护理工作的无价值感;面对绝症患者的无助时,个人的移情体验.结论 巴林特小组可以帮助临床护士识别自身压力源,并能提高其不良情绪的表达与自我觉察能力,对工作压力具有改善作用.

Qualitative Study on Experience of Nurses with Occupational Stress

Objective Based on analysis of Balint group cases,to unearth the negative feeling presented in nurses and to explore effective ways to deal with occupational stressor.Methods 10 nurses with over moderate degree occupational stresses were trained in a Balint group in their own hospital respectively at the same time with 6 sessions.A grounded theory-based approach with thematic content analysis of the field notes was used.Results 12 cases were presented.3 main themes were identified,including how to face incurable patients,conflicts between nurses and guardians of the patients,personal occupational definition,value conflict when facing terminal patient.Conclusion Balint group can help nurses to identify their occupational stressor from themselves and patients,and it is an effective way used to outlet nurses' negative feelings.At the same time,Balint group can enhance nurses' ability of self-awareness and contribute to the improvement of nurses' occupational stressor as well.
