双管喉罩在基层医院心肺复苏中的应用

目的 探讨双管喉罩在基层医院急救心肺复苏(CPR)中的应用价值.方法 选择2家基层医院2014年9月-2016年7月期间突发心跳、呼吸骤停的患者61例,随机分为A、B两组,A组31例,采用双管喉罩通气,B组30例,采用面罩通气,比较两组气道开放时间,5 min、10 min、20 min时的Sp02≥90％的例数,30 min时心肺复苏成功的例数以及两组并发症的发生例数.结果 B组气道开放时间明显短于A组(P＜0.05),A组气道开放后5 min、10 min、20 min Sp02 ≥90％的例数均高于B组,A组30 min心肺复苏成功例数高于B组(P＜0.05).A组患者的胃膨胀发生率、胃内容物误吸率及气道漏气率均明显低于B组(P ＜0.05).结论 双管喉罩操作简单,通气效果良好,且并发症少,安全性高,适合于在基层医院中推广使用.

Application of Proseal Laryngeal Mask Airway in Cardiopulmonary Resuscitation in Primary Hospital

Objective To investigate the application value of proseal laryngeal mask airway in cardiopulmonary resuscitation(CPR) in primary hospital.Methods From September 2014 to July 2016,a total of 61 cases of patients with sudden cardiac and respiratory arrest in two primary hospitals were randomly divided into 2 groups:the proseal laryngeal mask airway group (group A) and the bag-valve mask ventilation(BMV) group (group B).The airway opening time,the number of the patients with SpO2 ≥90％ at the time points of 5min,10min and 20min and of those being successfully resuscitated at the time point of 30min were compared between the two groups.The complications were also observed in the two groups.Results The airway opening time of group B was significantly shorter than that of group A,the number of the patients with SpO2 ≥90％ at the time points of 5min,10min and 20min and of those being successfully resuscitated at the time point of 30min in group A were higher than that in group B.The incidence rate of the complications such as gastric dilation,gastric contents aspiration and airway leakage in group A was significantly lower than that in group B.Conclusion The proseal laryngeal mask airway is simple to be operated and it can lead to good ventilation effect.It is safe and has less complication.This implement can improve the success rate of cardiopulmonary resuscitation and it is worth to be promoted in primary hospital.
