胃癌根治术后患者自我管理效能感与营养状况的相关性

目的探讨胃癌根治术后患者自我管理效能感与营养状况的相关性,为促进胃癌患者术后生活质量的提高提供依据。方法采用方便抽样法,选取2016年1-11月上海市某三级甲等医院127例胃癌根治术后6个月以上的患者为研究对象,采用中文版癌症自我管理效能感量表、患者主观全面评价量表(patient-generated subjective global assessment,PG-SGA)和实验室检查法对其进行调查。结果本组患者可疑营养不良及营养不良发生率为15.7%~46.5%,自我管理效能感总分为(83.25±20.73)分。根据PG-SGA评分结果,不同营养状况的患者在自我管理效能感总分、正性态度维度和自我减压维度上的差异均有统计学意义(均P0.01)。自我管理效能感总分与PG-SGA评分、白蛋白和前白蛋白水平相关(均P0.01)。结论医护人员应积极引导胃癌根治术后患者进行自我管理,增强其自我效能感,以期改善康复期的营养状态。

Study on the Relationship Between Self-care Efficacy and Nutritional Status of Postoperative Gastric Cancer Patients

Objective To explore the relationship between self-care efficacy and nutritional status of pa-tients with postoperative gastric cancer,and to provide the theoretical basis for promoting their quality of life.Methods Totally 127 gastric cancer patients with gastrectomy more than 6 months in a Class Ⅲ Grade Ⅰ hospital of Shanghai were selected by convenience sampling and investigated with Chinese version of strategies used by people to promote health (C-SUPPH),patient-generated subjective global assessment (PG-SGA)and laboratory indicators from January to November 2016.Results The rate of potential mal-nutrition and malnutrition ranged from 15.7% to 46.5%,and self-care efficacy scored (83.25±20.73).Ac-cording to the results of PG-SGA,the statistical significances of self-care efficacy and its subscales (positive attitudes and stress reduction)were found between patients with different nutritional status (all P<0.01). There was a correlation between self-care efficacy and PG-SGA,serum albumin,prealbumin,respectively (all P<0.01).Conclusion Medical staff should actively guide self -management project of postoperative gastric cancer patients,and strengthen their self-efficacy,in order to improve the nutrition during stage of rehabilitation.
