彩色超声联合弹性成像在甲状腺TI-RADS 4级病变中的应用价值

目的探讨彩超联合超声弹性成像在甲状腺TI-RADS 4级病变良恶性鉴别诊断中的应用价值。方法选取甲状腺常规彩超TI-RADS 4级评分的结节患者65例(共87个结节),均行超声弹性成像检查,并与病理结果进行对照分析。结果术后病理结果 87个甲状腺TI-RADS 4级评分结节中良性病灶28个,恶性病灶59个;单纯应用彩超良恶性鉴别诊断的敏感性、特异性、准确性分别为62.7%、75.0%和66.7%,彩超联合超声弹性成像评分良恶性鉴别诊断的敏感性、特异性、准确性分别为93.2%、89.3%和92.0%,均明显高于常规彩超检查,差异有统计学意义(P0.05)。结论彩超联合超声弹性成像可显著提高甲状腺TI-RADS 4级病变良恶性鉴别诊断的准确率。

Applicability of Color Ultrasound Combined with Elastography in Differentiating Between Benign and Malignant TI-RADS 4 Thyroid Lesions

Objective To assess the value of color ultrasound combined with elastography in differentially diagnosing benign and malignant TI-RADS 4 thyroid lesions.Methods Color ultrasound combined with elastography was performed in 87 TIRADS 4 thyroid lesions,which were diagnosed by conventional color ultrasound.The pathological results were compared.Results The sensitivity,specificity and accuracy of color doppler ultrasonography in diagnosis of thyroid TI-RADS 4 lesions were 62.7％ 、75.0％ and 66.7％,respectively while those of the combined approach were 93.2％,89.3％ and 92.0％ respectively.There was significant difference in the accuracy of diagnosis between the two groups (P＜0.05).Conclusion Color ultrasound combined with elastography imaging can improve the accuracy of diagnosis of TI-RADS 4 thyroid lesions.
