LigaSure在腹腔镜脾切除术中的应用

目的探讨LigaSure在腹腔镜脾切除术中的应用价值。方法回顾性分析南昌大学第一附属医院2007年5月-2008年5月42例接受腹腔镜脾切除术病人的临床资料，其中完全腹腔镜脾切除术14例、手助腹腔镜脾切除术28例。术中应用LigaSure游离脾脏及处理直径〈7mm的脾蒂血管。结果42例手术全部成功，无中转开腹，术后无并发症。平均手术时间为150min（60-260min），术中平均出血量为190mL（5～1500mL），平均脾重为730g（150～2025g），术后平均住院天数为8．3d（4～19d）。结论LigaSure可避免意外损伤，缩短手术时间，减少体内异物存留，可靠、有效地处理脾周韧带和血管。

Laparoscopic Splenectomy Using LigaSure

Objective To evaluate the advantage of using LigaSure in laparoscopic splenectomy （LS）. Methods The clinical data of 42 cases undergoing LS between May 2007 to May 2008 were retrospectively analyzed, including 14 totally laparoscopic splenectomy and 28 hand-assisted laparoscopic splenectomy. The entire dissection of the spleen and hilar vessels sealing （less than 7 mm diameter） were performed with liagasure. Results LS was successfully completed in all patients. There were no conversion to open surgery. There were no complications in the postoperative period. Mean operating time was 150 min （range,60-260 min） and mean blood loss was 190 mL（5- 1 500 mL）. Mean splenic weight was 730 g（rang,150-2 025 g） and mean postoperative hospital stay was 8.3 days（rang,4-19 d）. Conclusion LS using LigaSure could avoid accidental injury, shorten the operative time,reduce residual clips, and provide sufficient hemostasis.
