周围神经鞘瘤的CT和MRI分析

目的:分析周围神经鞘瘤的CT和MRI征象及病理组织学表现.方法:回顾性分析我院经手术病理证实的周围神经鞘瘤30例的CT和MRI征象及病理表现.结果:病变发生在颈部10例、胸部6例、腹部7例(含胃部4例)、四肢7例.30例周围神经鞘瘤中25例为良性,5例为恶性.20例行CT检查,其中17例良性病灶平扫密度较均匀,4例可见点状钙化灶,增强扫描病灶呈轻至中度强化,3例恶性病灶瘤体巨大,囊变、坏死多发,增强扫描强化不均匀.10例周围神经鞘瘤行MRI检查,MRI表现:T1WI病灶呈等或稍低信号,T2 WI呈高信号,增强扫描不均匀强化,10例病灶中,2例为恶性周围神经鞘瘤并伴淋巴结转移.结论:周围神经鞘瘤的CT和MR1表现具有特征性,CT和MRI能准确地显示肿瘤形态、部位、内部结构及与邻近结构的关系,对诊断有重要帮助,对手术治疗具有重要指导作用.

CT and MRI diagnosis of peripheral neurilemoma

Objective：To investigate the CT and MRI features of peripheral neurilemoma and pathological features.Methods：The CT and MRI leatures of 30 patients in our hospital with histopathology proved peripheral neurilemoma were retrospectively analyzed.Results：10 cases were located in cervic,6 cases in pectoral,7 cases in ventral（4 cases located in gastric）,and 7 cases in appendicular.Of the 30 tumours,25 were benign,5 were malignant.20 patients were performed CT scan,homogeneous density were performed with CT plain scan in 17 benign lesion,4 cases with calcification dots,which,on contrast-enhanced CT,showed low degree enhancement.3 malignant lesion were performed large area,with liquefaction necrosis,uneven enhancement were performed with contrast-enhanced CT scan.10 patients underwent MRI scan.On MRI,the tumor was of slightly hypo or iso-hypo signal intensity on T1WI and hyper signal intensity on T2WI,on contrast-enhanced scan,the lesion showed inhomogeneous enhancement,2 malignant cases were with lymphatic metastasis.Conclusion：Peripheral neurilemoma has characteristic features on CT and MRI.CT and MRI can accurately demonstrate the shape,location and internal structure of the lesion,which are valuable in the diagnosis of peripheral neurilemoma,providing valuable informations for surgical plans.
