后腹腔镜与开放性肾囊肿去顶术的疗效比较

目的:评价经后腹腔镜与开放性肾囊肿去顶术的治疗效果,探讨术中进入后腹腔的方法。方法:14例单纯性肾囊肿行后腹腔镜手术,其中11例采用直接闭合式方法进入后腹腔,21例行开放性手术。对两种术式的临床疗效、术后恢复等进行对比研究。结果:两种手术方式均能达到良好的肾囊肿去顶效果,后腹腔镜手术在术中出血,术后下床活动时间,术后住院天数,术后镇痛药使用情况方面优于开放性手术(P<0.05)。后腹腔镜手术1例中转开放性手术。结论:后腹腔镜肾囊肿去顶术疗效可靠,手术创伤小,术后恢复快。采用直接闭合式方法进入后腹腔,操作更简便。

Efficacy comparison between retroperitoneoscopic and open surgical unroofing of renal cyst

Objective:To evaluate the efficacy of retroperitoneoscopic and open surgical unroofing of renal cyst for exploring the method of entering retroperitoneum intraoperation.Methods:Retroperitoneoscopic was applied in 14 cases,11 of which adopted straight and closing method of entering retroperitoneum.Open surgery was applied in 21 cases.Curative effects and postoperative recovery state were compared.(Results:)Both methods could better result in removing renal cyst.Bleeding volume intraoperation,time movement out of bed postoperation ,days in hospital postoperation,analgetic used postoperation of retroperitoneoscopic were better than those of open surgery(P<0.05).One case of retroperitoneoscopic was turned to open surgery.Conclusion:Retroperitoneoscopic unroofing of renal cyst is safe and effective for its light trauma and quick recovery.It is easier to enter retroperitoneum by the straight and closing method.
