人工全髋关节置换术在髋臼骨折中的应用

髋臼骨折常继发于高能量暴力。髋臼骨折的常见并发症有创伤性髋关节炎、股骨头缺血性坏死、异位骨化。当出现髋关节疼痛及功能障碍时，对于部分患者实行人工全髋关节置换是惟一有效的手术方法。髋臼骨折 无论是非手术治疗还是手术治疗，必然会增加全髋人工关节置换术的难度。在人工全髋关节置换术中，选择切口、内固定和处理异位骨、骨缺损和骨不连、选择假体、控制感染和保护坐骨神经等都是应该注意的问题。无论是早期急诊还是髋臼骨折手术失败后，抑或是髋臼骨折晚期，人工全髋关节置换在髋臼骨折治疗中是行之有效的方法。

Application of total hip replacement after acetabular fracture

Fractures of the acetabulum are the result of high energy trauma. Fractures of the acetabulum can lead to degenerative of the hip, avascular necrosis of the femoral head and heterotopic ossification. when significant joint changes and pain are present, total hip replacement(THR) is the only reasonable method of surgical treatment for some patients. THR after non operatively or operatively treated acetabular fractures has more technical problems than routine primary THR. Important consideration include surgical approach,management of hardware and heterotopic ossification and management of defect and nonunion, option of component, management of infection and sciatic nerve in THR. It is concluded thatTHR acts an important role in the management of acetabular fractures, including acute management for certain rare cases, failed operatively treatment of acetabular and later acetabular fracture.
