十二指肠胃食管反流合并吸入性肺炎的支气管肺泡灌洗液分析

目的了解十二指肠胃食管反流（DGER）合并吸入性肺炎（AP）时的支气管肺泡灌洗液（BALF）的变化。方法实验组和对照组幼兔各5只，将胃液和胆汁注入实验组幼兔的气管内，复制成动物模型。对两组行支气管肺泡灌洗术，并对BALF的细胞和非细胞成分进行检测。结果实验组BALF的细胞总数明显增多（P〈0．001），以中性粒细胞为主（P〈0．001），巨噬细胞体积增大、并充满脂质。总蛋白明显增多（P〈0．001）。结论DGER合并AP时，胃酸和胆汁酸对肺脏有直接损伤作用；炎性细胞和炎症介质有间接损伤作用。BALF的改变有助于本病的诊断．

Analysis of Bronchoalveolar Lavage Fluid in DGER with Aspiration Pneumonia

Objective To understand the change of bronchalveolar lavage fluid （BALF） in duodeno- gastroesothageal reflux （DGER） with aspiration pneumonia （AP）. Methods Every group, experiment and control, there were five young rabbits. The experiment group were made of disease model by injecting gestric juice and bile in their trachea. Take bronchalveolar lavage, and test cellular and non - cellular composition in BALF, in both group. Results The change of BALF in experiment group as follow：The cell count increased obviously（ P 〈 0.001 ）, the neutrophil were in majority（ P 〈0.001）, the macrophages were macrosis and laden with lipid.The protein content increased too （ P 〈 0.001）. Conclusion In DGER with AP, the gastric acid and biliary acid there were directly injury action to it, the inflammation cell and cytokine there were indirectly injury action. The change of BALF were usefully diagnosis of this disease.
