ELISA法在梅毒螺旋体抗体检测中的临床意义

目的探讨基因重组抗原ELISA法在梅毒螺旋体抗体检测中的临床意义。方法用甲苯胺红不加热血清实验（TPPA）对355份梅毒患者和非梅毒患者的临床血清标本进行检测。结果以TPPA阳性者为标准,TRUST有7份为假阴性,4份为假阳性;TRUST和ELISA的敏感性、特异性分别为92.7%、98.3%和100%、99.1%。TRUST与TP-PA的符合率为96.3%,ELISA与TPPA的符合率为99.4%。ELISA的敏感性、特异性都优于TRUST,与TPPA符合率高。结论重组抗原ELISA法检测梅毒螺旋体特异性抗体具有敏感性高、特异性好、结果客观、自动化程度高等优点,适用于梅毒初筛和确诊。

Clinical significance of detection of antibody to Treponema pallidum by ELISA

Objective To evaluate the clinical sigificance of recombinant antigens-based enzyme-linked immunosorbent assay (ELISA) technique in detecion of antibodies against Treponema pallidum. Methods Three hundred and fifty-five clinical serum samples from persons with syphilis or without syphilis were detected by the Toluidine red unheated serum test (TRUST), the T.pallidum particle agglutination(TPPA) assay and the recombintant antigen-based ELISA,respectively. Results Using the TPPA as a standard method, 8 samples were found to be false positive and 4 were false negative by the TRUST. However, only two samples were false positive and no false negative was observed by the ELISA. The overall specificity and sensitivity of TRUST was 92.7% and 98.3%, respectively. The specificity and sensitivity of ELISA were 100% and 99.1%, respectively. The agreement between TRUST and TPPA was 96.3%. The agreement between TRUST and TPPA was 99.4%. ELISA was more sensitive and specific than that of TRUST and ELISA was highly coincident with that of TPPA. Conclusion The recombinant antigen-based ELISA technique is a sensitive, specific, objective and easily automated method, suitable for screening and confirmatory diagnosis of syphilis.
