外周血清CA125水平对慢性心力衰竭患者再住院率的预测价值

摘    要:目的探讨外周血清糖类抗原125(CA125)水平对慢性心衰(CHF)患者再住院率的预测价值。方法145例慢性心衰患者根据心功能NYHA分级标准分为Ⅰ/Ⅱ、Ⅲ、Ⅳ级三组,比较各组CA125和B型钠尿肽(BNP)水平。根据不同外周血清CA125、BNP水平分组,随访1~6个月,以死亡或再住院为终点事件,分析不同外周血清CA125、BNP水平慢性心衰患者的再住院率。结果血清CA125水平随NYHA心功能分级增高而增高Ⅰ/Ⅱ级(18.79±5.23)U/ml,m级(82.34±24.37)U/ml,Ⅳ级(187.40±37.42)U/ml,P〈0.05];慢性心衰患者再住院率随CA125水平增高而增高(P=0.011),随BNP水平增高而增高(P=0.001)。CA125联合BNP对慢性心衰患者再住院率的预测价值更好(P=0.00012)。结论血清CA125水平不仅能反映心衰的严重程度,还能反映慢性心衰患者的预后。

The predictable between CA125 and the rehospitalization rate in CHF patients

Abstract:Objective To study the relationship between CA125 and the rehospitalization rate in CHF patients. Methods 145 patients with CHF were divided into three groups based on the standard of NYHA classification. The subgroup were divided according CA125 levels(35 U/ml) and BNP levels(400 pg/ml). After 1- 6 months follow-up, the endpoints, mortality and CHF rehospitalization were analyzed according the level of CA125 and BNP. Results The higher the serum of CA125, the higher NYHA functional class Ⅰ /Ⅱ (18.79± 5.23)U/ml, Ⅲ (82.34±24.37)U/ml, Ⅳ (187.40±37.42)U/ml, P〈0.05], the rate of rehospitalization in patients with CHF was higher with the increase of serum CA125 (P=0.011 ) and BNP(P=0.001 ). The combination CA125 with BNP showed more accurately predictable value ofCHF rehospitalization (P=0.00012). Conclusion Serum levels of CA125 are associated with the severity of CHF, which can prognosis the rehospitalization in CHF patients.
