蒙药透骨灵橡胶膏治疗膝骨性关节炎的临床疗效及对相应指标的影响

目的:通过随机对照试验,探讨以蒙药透骨灵橡胶膏治疗膝骨性关节炎的临床疗效,为治疗提供临床实验依据。方法:选取膝骨性关节炎患者63例,利用随机数表法分为三组,每组21例。透骨灵橡胶膏组采用蒙药透骨灵橡胶膏治疗,骨通贴膏组采用骨通贴膏治疗,盐酸氨基葡萄糖组采用盐酸氨基葡萄糖片治疗。治疗周期28 d。记录并统计分析治疗前、治疗2周后、治疗4周后、随访4周的WOMAC量表,蒙医症候及临床症状体征记分,VAS评分表以及SF-36生存质量评估表,同时治疗前后用ELISA法检测血清中CGRP、TNF-a、IL-10浓度。结果:透骨灵橡胶膏组治疗前后VAS评分、SF-36表、WOMAC量表评分有显著下降(P<0.05)。透骨灵橡胶膏组总体疗效与骨通贴膏组和盐酸氨基葡萄糖组相近,疼痛方面的疗效优于骨通贴膏组和盐酸氨基葡萄糖组。治疗后,TNF-α、CGRP浓度均有显著降低(P<0.05),IL-10的浓度有上调趋势。结论:蒙药透骨灵橡胶膏治疗膝骨性关节炎疗效佳,能够止痛、消肿、改善骨关节功能,抑制血清中TNF-α、CGRP表达(P<0.05),上调IL-10浓度,减轻炎性反应。

Clinical efficacy of Mongolian medicine Touguling Rubber Ointment in the treatment of knee osteoarthritis and Its influence on corresponding indicators

Objective:To explore the clinical efficacy of Touguling Rubber Ointment(TGL)in the treatment of knee osteoarthritis by randomized controlled trial and to provide clinical experimental basis for the treatment of knee osteoarthritis.Methods:Sixty-three patients with knee osteoarthritis were randomly controlled.The treatment group was treated with Mongolian medicine Touguling Rubber Ointment,while the control group was treated with Gutong Plaster and Glucosamine Hydrochloride tablets with 21 cases in each group.The treatment period was 28 days.The clinical efficacy was evaluated by WOMAC scale,Mongolian medicine syndromes and clinical symptoms and signs score,VAS scale and SF-36 quality of life evaluation table before treatment,2 weeks after treatment,4 weeks after treatment and 4 weeks after follow-up.At the same time,the serum levels of CGRP,TNF-a and IL-10 were measured by ELISA before and after treatment.Results:The VAS score,SF-36 form and WOMAC score of Touguling Rubber Ointment group decreased significantly before and after treatment(P<0.05).Compared with the control group,the overall effect of Touguling group was close to that of the two control groups,and the pain effect was better than that of the control group.After treatment,the concentration of TNF-a and CGRP decreased significantly(P<0.05),while the concentration of IL-10 increased.Conclusion:The Mongolian medicine Touguling Rubber Ointment has a good effect on treating the patients with knee osteoarthritis,and it can relieve pain,detumescence and improve the function of bone and joint with statistical significance(P<0.05).It can inhibit the expression of TNF-alpha and CGRP expression in serum(P<0.05),up-regulate the concentration of IL-10,and alleviate inflammatory reaction.
