北京城市居民脑卒中类型与血压的关系

选择北京市崇文、西城两地自然人群共19546人,对年龄35岁以上者,整群随机调查5441人进行前瞻性观察.本组结果脑卒中四年累积发病者181例,CT检查114例,占63%,在混合性高血压和单纯收缩期型高血压患者中并发脑卒中者,出血性占81%,缺血性占68.8%(P<0.01).收缩期型高血压发生卒中者,老年组(年龄60岁以上)高于非老年组(P<0.01),收缩压的升高是引起老年人中风的重要危险因素.脑卒中CT分类,脑出血占33.3%;脑梗塞占60.5%,有29.2%正常血压者发生缺血性卒中.因此积极寻找血压之外密切相关的危险因素也是降低缺血性卒中不容忽视的重要环节.

THE ASSOCIATION BETWEEN THE TYPE OF STROKE AND BLOOD PRESSURE IN BEIJING URBAN RESIDENTS

A population of 5441 aged 35 years and over was selected from 19546 residents of Chong Wen and West Cheng districts in Beijing for the prospective study. The result showed that the incidence of stroke was 151; of them the diagnosis of 144 cases (63%) were proved by brain CT. Among stroke patients with mixed type of hypertension and isolated systolic hypertension cerebral hemorrhage accounted for 81%, ischemic stroke 68.8% (p<0. 01). And stroke patients with only isolated systolic hypertension were more in old age group (over 60 years) than in other age group. Increase in systolic blood pressure was a major risk factor for stroke in old age group. The type of stroke identified by brain CT was as follows, 33% of stroke cases were diagnosed as cerebral hemorrhage, 30. 7% cerebral infarction, 29.8% cerebral thrombosis. The levels of blood pressure in 29. 2% of patients with ischemic stroke were normal. It is an important task to find out risk factors other than high blood pressure for decreasing ischemic stroke.
