环磷腺苷葡胺、1,6-二磷酸果糖联合治疗小儿毛细支气管炎合并心肌损害疗效观察

摘    要:目的观察环磷腺苷葡胺(meglumine adenosine cyclophosphate,MAC)、1,6-二磷酸果糖(fructose l,6-diphosphate,FDP)联合治疗小儿毛细支气管炎合并心肌损害(bronchiolitis complicated with myocardial damage)的疗效,探讨最佳治疗方案。方法将2007年1月至2009年6月山东省济南市第三人民医院收治的毛细支气管炎合并心肌损害的235例患儿随机分为治疗组(n=115)和对照组(n=120)(本研究遵循的程序符合山东省济南市第三人民医院人体试验委员会制定的伦理学标准,得到该委员会批准,分组征得受试对象监护人的知情同意,并与其签署临床研究知情同意书)。两组患儿除采用抗感染、吸氧、解痉、平喘等常规综合治疗之外,治疗组再采用环磷腺苷葡胺、1,6-二磷酸果糖联合治疗营养心肌;对照组则加能量合剂营养心肌。观察两组主要临床症状消失时间、心肌酶谱变化。结果治疗组患儿临床症状、体征消失时间和血清心肌酶恢复时间,均明显短于对照组,差异有显著意义(P<0.01)。结论环磷腺苷葡胺、1,6-二磷酸果糖联合治疗毛细支气管炎合并心肌损害疗效明确,未见明显不良反应,但是否值得临床推广应用,尚需多中心、大样本随机对照研究证实。
