复合羟基磷灰石进行根管充填治疗的实验研究

摘    要:目的 探索一种理想的根充剂,使在根周病治疗中根管充填更为理想。方法采用NPHA及与BMP组成NPHA—BMP复合物进行动物实验。以NPHA和NPHA—BMP复合物进行家兔肌内植入实验,羟基磷灰石(HA)及NPHA则分别植入家兔胫骨内的成骨效应实验。结果动物实验组织学观察证实:NPHA植入肌肉未见诱导幼稚间充质细胞的增殖、分化以及成骨作用,但该材料不引起组织排斥反应,生物相容性良好。NPHA—BMP复合物植入肌肉内,则见明显新生骨形成,新生骨成长活跃且与材料呈直接界面,说明NPHA—BMP具有诱导成骨活性的作用;NPHA植入胫骨内观察可见无免疫排斥反应,其生物相容性良好,内联、多孔样结构有利于传导成骨,其成骨量明显多于致密性羟基磷灰石。结论NPHA具有类似人骨的网状交通孔隙,有利于传导成骨,将其与BMP复合,组成NPHA—BMP复合物,经动物实验证实,该材料生物相容性良好,具有传导和诱导成骨功能,是当前根管充填术中理想的充填剂。

Abstract:Objeeetive To explore an ideal root-fill material. Methods The natural porous hydroxyapatite (NPHA) and hone rnorphogenesis protein (BMP) were combined as NPHA- BMP complex. Animal experiment: NPHA- BMP complex was implanted into the rabbits' muscles to evaluate its ostcoinduce effect. Hydroxylapatite (HA) and NPHA were implanted into the rabbits'shankhoes respectivdy to compare their osteoconductivity. Clinical observation: The patients were randomly selected to the experiment group ( NPHA - BMP group) and the control group( Ca(OH)2 group), The root canal therapy were operated with NPHA- BMP and Ca(OH)2 respectively. Results The animal experimental histopathological observation showed that NPHA which implanted into the rabbits' muscles can' t induce the multiplication and polarization of the infantile mesenchyme cell and hasn't osteoconductibity. But NPHA has gcxxt biocompatibility. After the NPHA- BMP was implanted into the rabbits' muscles, new bone was formed obviously and has a direct interface with the comlex. The experiment of NPHA implanted into rabbits' shankhones showed that the communicated, porous structure is valuable for osteoconconductivity. NPHA can induce more new hone than the intensive hydroxylapatite. One year follow up showed that the NPHA - BMP was more efficacious than Ca(OH)2 significantly. Conclusion NPHA has more valuable for osteoconductivity. The animal experiment and clinical observation exhibited NPHA- BMP has excellent biocompatibility and ostcoinduce effct and osteoconductivity. So the NPHA/BMP complex should be an ideal root- fill material.
