脑卒中急性期血压变化与其预后关系研究

目的探讨脑卒中急性期患者的血压变化对预后的影响。方法于发病第1、3、7日观察110例急性脑卒中患者(其中脑梗死76例,脑出血34例)血压变化趋势,同时评定第1、3、7日及第30日神经功能缺损程度评分及病残程度分级。结果急性脑卒中患者入院后,首次血压增高者占87.3%,而动态血压较随测血压值低。在无特殊降压措施下,血压随时间逐渐下降;生活自理组与生活依赖组脑卒中后血压增高幅度比较差异无显著性。结论脑卒中急性期血压存在自发下降趋势,脑卒中急性期高血压的治疗需慎重、合理,治疗宜个体化。

The Clinical Study on Ambulatory Blood Pressure Change and the Correlation with The Prognosis in Acute Stroke

alObjective:To evaluate ambulatory blood pressure change and the prognosis value in acute stroke patients.Methods:A total of 110 patients (cerebral infarction 76 cases, cerebral hemorrhage 34 cases) were selected within 12h after acute stroke onset corresponding with the diagnostic criteria of acute storke. All patients were assessed within 24h onset by same observer with causal and ambulatory blood pressure monitoring, MESs and scale of handicap. Repeated assessment were made at the 3rd, 7th day and the 30th day after onset. Results:The blood pressure grows up in 87.30% of patients at the first assessment after acute stroke onset,and the ambulatory blood pressure is lower than the causal one. The evaluated blood pressure after acute stroke declined in the following days, without specially daministrative drug. There were no significant difference in blood pressure levels between the life independent group and life dependent group. Conclusion:The management of evaluated blood pressure during acute stroke should be cautious appropriate and individualized.
