实时荧光定量RT-PCR检测肺癌、癌旁组织及外周血中CK19 mRNA的表达及临床意义

摘    要:目的 探讨角蛋白CK19在肺癌患者癌和癌旁组织及外周血循环细胞中的表达特点及其临床意义。方法 采用实时荧光定量RT-PCR的方法对62例肺癌患者癌和癌旁组织内CK19mRNA表达进行检测,并与其对应的外周血中的CK19 mRNA表达结果进行比较。结果 62例肺癌患者的癌和癌旁组织和外周血中CK19mRNA均有表达(分别为61.2%、32.2%、40.3%),而肺癌组织中CK19mRNA水平明显高于癌旁组织,两者差异显著(P〈0.05)。外周血中CK19mRNA的表达与临床分期和病理类型无关;却与肺癌组织中CK19mRNA的高表达水平及淋巴结是否转移密切相关;健康对照组则无阳性。结论 实时荧光定量RT-PCR方法检测肺癌患者外周血中CK19 mRNA表达有较高的敏感性和特异性,且操作方便,在肿瘤微转移的诊断监测方面有一定的应用价值。

Abstract:Objective To investigate the expression of CK19 mRNA incancerous and paracancerous tissues and peripheral blood cells in patients with lung cancer and its clinical significance.Methods The expression of CK19 mRNA in cancerous and paracancerous tissues and peripheral blood cells of 62 patients with lung cancer were detected by real-time fluorescent quantitative RT-PCR and compared with those values in peripheral blood.Results The positive expression rates of CK19 mRNA in cancerous and paracancerous tissues and peripheral blood cells were 61.2%,32.2%,and 40.3%,respectively,showing a significant difference between tumor tissues and tumor-surrounding tissues(P<0.05).The expression pf CK19 mRNA in peripheral blood was not related to the clinical staging or phathological type,but was closely related with the high expression of CK19 mRNA in lung cancer tissues and lymph node metastases.CK19 mRNA was not expressed in the lung tissue of healthy controls.Conclusion Detection of the CK19-mRNA expression in peripheral blood by real-time fluorescent quantitative RT-PCR has a high sensitivity and specificity in patients with lung cancer.It is easily performed and may be applied in the diagnosis and monitoring the tumor micrometastasis.
