幕上原始神经外胚瘤的诊断与治疗(附5例报告)

目的 探讨幕上原始神经外胚瘤（SPNET）的临床特点及治疗效果。方法 总结5例SPNET病人的临床资料，影像学表现、病理特点及治疗结局。行急诊手术2例，常规手术3例，术后3例全脑放疗，2例联合化疗。结果 颅内病灶全切除3例，部分切除2例，随访4-18个月，死亡1例，2例无病情进展，2例进展。结论 SPNET是一种恶性程度极高的肿瘤，对化疗和放疗较敏感。

Diagnosis and treatment of supratentorial primitive neuroectodermal tumor: report of 5 cases

Objective To study clinical characteristics and treatment outcomes of supratentorial primitive neuroectodermal tumor (SPNET). Methods The clinical findings, imaging features, pathological results and treatment outcome of 5 patients with SPNET were retrospectively reviewed. All the patients underwent microsurgery, and two out of the 5 patients underwent emergency operation. Three patients received conventional cranial radiation, and the other 2 underwent radiotherapy combined with chemotherapy after the surgery. Results The lesion was totally removed in 3 patients, and partially incised in 2. The patients were followed up for 4 to 18 months. One patient died in the ninth month after operation. Two patients gained a constant remission, and two other deteriorated. Conclusion SPNET is one of the high-grade malignant intracranial tumors, and sensitive to the radiotherapy and chemotherapy.
