摘    要:在一个街区选择两组35岁以上血压≥18.67/12.00kPa 人群进行干预高血压人群的前瞻性研究。对干预组(1996人年)进行随访观察并采取干预措施,对照组(2283人年)不给特别的干预与保健措施,观察两组脑卒中及其它死因的死亡情况。结果干预组脑卒中死亡率显著低于对照组(RR=0.47 y,P<0.05)全死因死亡与冠心病死亡虽然干预组也低于对照组,但无统计学显著性,而除去脑卒中、冠心病之外的其它死因死亡率两组基本一致(RR=1.01)。两组脑卒中死亡率的差异按年龄、收缩压水平分层分析,以及以 Cox 回归模型调整几个因素的影响后,仍有统计学显著性。结果表明,在人群中干预高血压人群明显降低脑卒中的死亡率。

Abstract:An intervention study with two groups of hypertensions of 35 years old or over in a com- munity was carried out.Active follow-up and treatment were applied to cared group (1996 person-years were observed)and no special intervention and care offered to control group (2283 person-years were observed).The death of stroke,coronary heart disease and others were recorded.Compared with control group,the death rate of stroke in cared groups was signifi- cant lower (RR=0.47,p<0.05).As to the total death and CHD death,the rates bet ween two groups didn't differ significantly,though in the cared group the rates were lower.However, the death rates with other causes in two groups were very similar (RR=1.01).The difference of stroke mortality between two groups was still significant when it was analised with strat- ification according to age or systolic blood pres- sure,as well as with Cox modelling.It sugge- sted that,the intervention to hypertensions in the community be effective to decrease the stroke mortality.
