| 摘 要： | 目的探讨胆总管切开探查一期缝合不置T管引流的可行性、手术指征及临床价值。方法胆总管结石或可疑结石患者104例,随机分为两组:一期缝合组59例,胆总管探查术后不置T管引流,用4-0或5-0无损伤可吸收缝线一期缝合胆总管;T管引流组45例,行T管引流。结果术后并发症发生率两组无显著差别(P>0.05)。一期缝合组在术后首次肛门排气时间、抗生素使用时间、平均输液量、输液时间、下床活动时间、平均住院时间及住院费用等方面均优于T管引流组(P<0.05);随访6个月~3年,未发生胆管结石残留及胆管狭窄的并发症。结论只要正确掌握手术适应证及缝合技巧,胆总管探查后一期缝合是安全可行的,有推广应用价值。
