摘    要:目的:报告我科1986年7月~1996年7月经蝶显微手术治疗垂体腺瘤360例.女性233例.男性127冽:年龄13~66岁、平均38.6岁.其中大腺瘤143例(39.7%).微腺癌217例(603%).方法:均经CT扫描或MRI确诊.手术采取经唇下-鼻中隔-蝶窦入路或经鼻前庭-蝶窦人路两种方式行肿瘤切除术.结果:术目无死亡.296例获长期随访(平均3.5年).246例(83.1%)恢复良好.50例(16.9%)肿瘤有复发,需行再次手术、或采用药物、放疗或放射外科治疗.结论:垂体腺瘤采取经蝶显微外科治疗是一种安全、有效的方法.

Abstract:Objective:360 patients with pituitary adenoma were treated by transsphenoidal microsurgery in our department from July 1986 to July1996. There were 233 females and 127 males. The patients ranged in age from 13 to 66 years with a mean age of 38. 6 years. Of the patients, 143 (39. 7%) were macroadenomas and 217 (60. 3%) were microadenomas. Methods; The diagnosis of disease was made by CT or MRI scan. Operations were performed via either sub-labio-septo-sphenoidal approach or naso-vestibulo-sphenoidal approach. Results: There were no death after operation in this group. Long-term follow-up review (mean 3.5 years) in 296 patients revealed good recovery of 246 patients (83.1 % ). The overall late recurrence rate was 16.9% ,and that of macroadenomas and microadenomas were 13.2% and 3.7% respectively . Those patients with recurrence of tumor need to be treated again by operation or by medical therapy, or radiotherapy, either alone or combined. Conclusion; Microsurgical technique via transsphenoidal approach is a safe and effective method for treatment of pituitary adenomas.
